Department Undergraduate Forms
Activities pertaining to a student's education need to be documented. The forms used by the Department of Computer Science and Engineering to document any special considerations are listed below. These forms can be downloaded and printed out. Forms are to be submitted to the Department office (ENB 342), unless otherwise noted. Forms are not to be submitted as email attachments.
Forms for class registration are found on the Registration Procedures page.
- Academic Petition - used to request a substitution or special consideration for a requirement. The department academic petition is for department requirements only. An academic petition for late course add, drop, or change requires the university academic petition form which is administered by Engineering Advising.
- Contract form for Independent Study - used to establish a contract between the student and the supervising professor for an activity outside of a classroom course. IS will not substitute for a required class. The IS provides an opportunity for the student to do independent research or project in an area not typically covered by a department course. The student MUST have a contract signed by the supervising professor and approved by the department chair to register for IS.
- Contract form for Incomplete Grade - used to establish a contract between the student and the supervising professor for completion of an Incomplete ("I") grade.
- Contract form for Gate Course Enrollment - to be executed for every student enrolling in Computer Organization (CDA 3103) who has not completed the six key Departmental prerequisites at the date of registration.
- Graduation - used to apply for a Bachelor degree.

- Undergraduate academic regulations committee petition - used for readmission, late or retroactive drop, total withdrawal from entire term, late registration, late add, even exchange, and other petitions. For late adds, late or retroactive drops, and total withdrawals a completed Instructor’s Documentation Forms is required (available here).
